Book this option if you have already had your 1st, 2nd or Lab Review, perhaps some follow up visits and we are at the point of check ins.  Booking check in visits is totally up to you and you can take advantage of them as long as you wish and as long as you have had a recent visit within the last 90 days and there has been no significant change in your health condition or history that we have to go through.   Most of these are done virtually since they are 10-15 minute check-ins.

During your visits, Dr. Lelwica might recommend you work with one of the IFM certified coaches on our team.    This is a great way to get regular one on one assistance and advice with implementing your prescription and lifestyle changes.   It is also very cost productive for you.    If there are not times available to you on our website, reach out and we can make something happen that works for you!

This is where you start with your first visit with Dr. Lelwica.   After you book, you will be sent an email with the link to the intake forms.    In order to make the best use of your time with Dr. Lelwica, we ask that your intake forms are completed at least 2 days in advance of your scheduled visit.   Expect to plan on spending 1 - 1 1/2 hours doing the intake as Dr. Lelwica will want a full history from birth to now in order to facilitate getting to the root cause of your current health status.
