If the last names do not match, a birth certificate is also needed.Cartilage, Nose, Eyebrow, Lip, and Navel Piercing:Over 16 years old, with a valid passport, driver’s licence or government issued ID.Additionally, minors of 16 or 17 years of age must be accompanied by a legal guardian with a valid passport, driver’s licence or government issued photo ID. If the last names do not match, a birth certificate is also needed.Alternatively to being present at the studio, the legal guardian can fill a remote parental consent form (link on our website, or just get in touch please) and give a copy to the minor, along with a copy of the legal guardians ID. The minor of 16 or 17 years of age must then present this form, their ID and a copy of the legal guardians ID.
