This is usually for certify that you are who you said you are before you sign a legal Document in front said notary. More than one person can be on this acknowledgement.  The document can be provided if requested. I cannot recommend one to you. 

Customer affirms or swears on form that they are who they say they are to the notary. This document can be offered by request but I cannot recommend any set form you have to request the form you need.
