If the last names do not match, a birth certificate is also needed.Cartilage, Nose, Eyebrow, Lip, and Navel Piercing:Over 16 years old, with a valid passport, driver’s licence or government issued ID.Additionally, minors of 16 or 17 years of age must be accompanied by a legal guardian with a valid passport, driver’s licence or government issued photo ID.
