Parent’s/Coach’s/supervising adult Waiver and Medical Release Form : As the parent/ coach/supervising adult; By Booking a rental I hereby guarantee I have permission from the player/parent/guardian of each player/participant on that list to book the rental as the coach or supervising adult to conduct a rental/practice/training/lesson at The Cage(T.J. O’Donnell Baseball School, LLC) . Each player/participant has received a physical examination by a physician and has been found physically capable of participating in the Programs. I hereby have the authority to give my consent to have an athletic trainer and/or doctor of medicine or dentistry provide the participants with medical assistance and/or treatment and agree to be responsible financially for the cost of each assistance and/or treatment. I as the parent/ Coach/supervising adult, the undersigned, for ourselves, our heirs, executors, and administrators waiver, release and forever discharge The Cage, The T.J. O’Donnell Baseball School L.L.C., its staff, T.J. O’Donnell, Dan Calicari,officers, agents, representatives, employees, successors and assigns of and from any and all rights and claims for dangers resulting from injury to person or property which may be sustained or occur during participation in rental/clinic/lesson activities or arising from traveling to or from rental/clinics/lessons, whether said damages, injury or loss is due to negligence or not.
