*By scheduling this appointment to come into the
office and pack your desk, you are acknowledging that you have carefully
read all desk cleanout instructions (listed below as a reminder) and you are
agreeing to comply.Please arrive at the office during your scheduled time. You will
have 90 minutes to do the following:1. Pack up all personal
belongings. Boxes will be provided
near the entrances of the suites. Suite 200 boxes will be in the main
Suite 200 lobby.2. Pack up any office supplies that you need for
your home office. If you plan to take home any IT/office equipment from your
desk, YOU MUST submit a Help Desk Ticket for inventory tracking (Reminder: Only
1 monitor is allowed for home office. Do not take home
a second monitor). We will be tracking all inventory leaving the
building. For any items you do not need, please put in designated
boxes located in each suite. Please ensure all drawers and overheads are empty
before leaving your desk.3. Keeping PI top of mind, please shred any
files that you will not be taking home. As a reminder, foamboard posters,
binders, CDs, and flash drives cannot be shred. If you have any of these items,
please use the designated box for confidential items that cannot be shred. You
may use your trashcan for any non-confidential items.4. Carts will be available if needed and parked
near the entrances of the suites. If you use a cart, please return it to where
you found it. You may also bring someone to assist you with carrying items to
your car if needed. If you are moving a monitor, remember to lay it face down
on the cart and face down in your vehicle to avoid damage.5. Please use the disinfectant provided to wipe
down your desk before leaving. Cleaning supplies will be located next to shred
bins in all the suites.Additional Information and Reminders:· Per our DWA Employee Safety Training, please
cancel your appointment if you are not feeling well, have a temperature, or
have been exposed to COVID-19 in the last 10 days. This applies to anyone
assisting you in the building as well· If you believe you will need more than 90
minutes, please email Amanda Brooks or Dana Wallock· If you plan to take home the monitor on your
desk – and it is currently connected by a metal arm – please include this note in your IT
ticket so that they can prepare to disconnect the monitor from the arm before
your appointment (will require advance notice)· You will need your badge as well as your
building access code to enter the building. If you are unsure of your access
code, please email Amanda Brooks, Dana Wallock, or Terri Pratt· Be sure to bring your keys to your desk and
leave them at your desk once you have completed cleaning it out
· Please wear your mask and practice social
distancing while you are in the office
