IMPORTANT INFORMATION REGARDING YOUR APPOINTMENT:·        If you are married, you must submit a copy of your certified and recorded marriage certificate.   If you do not have this document, you may submit your pension application while you work to obtain it.  ·        You must provide proof of your date of birth and, if you are married, your spouse’s date of birth.  To prove date of birth, submit a certified copy of your (and your spouse’s) birth certificate.  Other documents may be acceptable for this purpose, contact the Trust Fund Office prior to your appointment.·        If you are divorced, you must provide a copy of your Dissolution of Marriage (divorce decree) for each divorce unless you have already done so. If a portion of your benefits were awarded to your ex-spouse, a Qualified Domestic Relations Order (QDRO) will also need to be provided.
