Adult Passport· Form DS-82 (Completed online and printed showing a barcode on the top left hand corner)· Current U.S. passport· (1) passport-sized picture (2”x2”, white background)· Fee of US$130.00, paid by credit or debit card only Note: If you are 16 years of age or older, and this is your first adult passport application (10-year validity), please follow the instructions notated below (*). Minor Passport (Under 16)Both parents listed on the birth certificate, along with the applicant, must be present. If one parent does not live in the Cayman Islands, he or she must submit Form DS-3053, which must be notarized by a U.S. Notary, along with a copy of the ID used by the notary.· Form DS-11 (not signed) - Completed online and printed showing a barcode on the top left hand corner· Current U.S. passport· Original birth certificate, Consular Report of Birth Abroad (if applicable) or Certificate of Naturalization (if applicable)· (1) passport-sized picture (2”x2”, white background)· If under age 8, sequence of age progression photos (one face shot per year from birth to present, which may be placed as a collage in a MS Word document)· Parents’ IDs (valid passport or driver’s license)· Fee of US$135.00, paid by credit or debit card only * If you are 16 years of age or older, and your current passport is either lost or stolen or valid for less than 10 years, the required documentation is the same as that listed above for a Minor Passport, with the exception that the fee is US$145.00. Applicants who are 16 or 17 years of age must appear in person with at least one parent who is listed on the birth certificate. Lost or stolen passport applications must also include Form DS-64 and a police report. Consular Report of Birth Abroad (CRBA)*CRBA appointments may not be booked using this booking link and must be placed on the wait list by contacting CaymanACS@state.gov.*A child born abroad to a U.S. citizen parent(s) may acquire U.S. citizenship at birth if certain statutory requirements are met. Parent(s) must demonstrate a biological relationship to the child as well as 5 years physical presence (2 years after the age of 14) in the United States prior to the child’s birth in order to transmit U.S. citizenship. Create an account at MyTravelGov Access eCRBA and submit your application online. The online process provide applicants with step-by-step instructions to complete the application. After completing the online application and fee payment, you will receive a confirmation email within 72 hours of fee payment.
