I require a $25 deposit to guarantee your appointment. I can allow up to 15 min grace period for scheduled appointment. Any longer we will have to reschedule. No guests are allowed to wait with you and Face Mask are Mandatory. I ask for a 48 hour notice for rescheduling or cancellations or deposit will be forfeited.

A memory screening by phone takes just 15 min. to complete, but we ask that you book a 30min. slot to allow time for us to go over the results with you.Please ensure that during your memory screening appointment, you are in a place that is quiet and calm to ensure that you are able to concentrate during the assessment.Amy will call you at the number you provided on your intake form at the appropriate time.
