1. Enter Building B
2. Take Elevator #5 to the 2nd Floor
3. After exiting the elevator, B248 will be located down the hall on the right

1. Enter Millberry Union 500 Parnassus Ave (food court/gym) from the East entrance.
2. Upon entering, take the first right. There is an elevator and stair set next to the gym entrance around the corner.
3. Scan your UCSF ID badge on the reader next to the stairs, and take them to the 2nd floor. If taking the elevator, scan your ID on the small glass panel and select floor 2. If you have trouble gaining access, call 415 502-7361.
4. Exit on the 2nd floor and proceed through the North door.
5. MU253 is all the way down the hall on the right.
6. If you are late or miss your appointment, please reschedule.

N95 Respirator Fit Testing
N95 Respirator fit testing appointments are for those who are at high risk for exposure to airborne infectious agents/materials.
FIRST, ensure that you have verified with your manager that you are assigned to perform tasks which place you at high risk for exposure to airborne infectious agents/materials. Please also ensure that your department does not already have a separate group session arranged.
Prior to attending a session, you must verify that your medical clearance is on file with Occupational Health Services (OHS). If you were fit-tested within the last 12 months, it is likely that you have medical clearance on file with OHS. If you are unsure, contact Occupational Health Services at 415-885-7580, or complete the OHS questionnaire, Respirator Clearance form, fax it to OHS at 415-514-5614 for clinical review, and wait 24-48-hours to attend one of the scheduled sessions.
Individuals must be clean shaven to be fit tested, and in general to wear an N95 or any other type of tight-fitting respirators; facial hair is only permitted if it does not cross the sealing surface of the tight-fitting respirator. For additional guidance on facial hair, please visit the NIOSH/CDC website.
If you need training for a Powered Air Purifying Respirator (PAPR/CAPR), please review:
a) UC Learning Center for PAPR Training Video (Log-in and search for PAPR), or
b) Manufacturer’s instructional videos available at UCSF Health Safety Office website.
After watching the training videos, schedule a regular appointment and complete the paperwork mentioned above. If needed, you may also perform hands-on training during your appointment. For PAPR fitting appointments at Mount Zion, please sign-up only for the last session of the day.
