COVID-19 Vaccinations are now available for scheduling! Click "Book Now" to book your turn in line. There are two options:1) 75th Med group This is the regular scheduled times2) 75th Med group stand by To minimize waste, and due to no shows, a standby list is required. Vaccines will be given to complete a vial. All of these individuals are not guaranteed to receive the vaccine, depending on how many doses we have left for the day. If on this list you will be contacted if vaccine is available for you that day.Visit the Resources page to:1) Review MODERNA Fact Sheets2) Complete the DHA Form 207, print and bring with you to the vaccination line (required)3) Register for v-safe so CDC can track symptoms you are having with the vaccine. It will send you reminders and ask you how you are feeling.
