What to bring...If you're renewing your passport (and its expiration date is under five years from today's date), you must bring the expired passport with you.If you've had a name change (through court order or marriage), you must bring documents supporting the name change and they must have a raised seal.In all cases (in applying for a new passport), you will need your Birth Certificate (original or document with a raised seal), documents supporting any name change (and/or divorce decree) if applicable. If you've been Naturalized, you will only need your Naturalization Certificate to prove citizenship.In all cases, if you've ever been issued a U.S. passport please, bring that passport with you as you must turn it in.
