About the company

The Southlake Community Ontario Health Team, with the support of Reena and YSSNis offering dedicated appointments to people with developmental disabilities and their caregivers. Please review the information below before proceeding to the booking page. 


Third/booster doses are available by appointment only.


If you need help booking an appointment, please call 905-898-6455 Ext. 6600. You may be asked to leave a message, and someone from YSSN will return your call. 


ELIGIBILITY 


A booster dose of the COVID-19 vaccine is available at least three months (84 days) after receipt of the second dose for the following eligible groups: 


⇒ Ontarians with a developmental disability who are 18 years of age or older; and,

⇒ Caregivers and Support Workers who reside in Ontario, are 18 years of age or older and care for someone with a developmental disability.

VACCINE AVAILABILITY 


⇒ Moderna - Only 30 years of age and older.

⇒ Pfizer - Only 18 to 29 years of age.


LOCATION 


Ray Twinney Recreation Complex- 100 Eagle St W in Newmarket


⇒ parking is located in the North parking lot, in front of the Multi-Purpose Room entrance. Signage will be available to lead you to the vaccination clinic parking.

⇒ the clinic's entry and exit are accessible for people with assistive devices. If you use a bariatric wheelchair, however, you will be directed to the main entrance. 


ON-SITE ACCOMMODATIONS


The clinicians can offer the following accommodations for people with behavioural or physical complexities:


⇒ in-vehicle vaccinations; and,

⇒ private rooms


If you require either accommodation, please have someone notify the check-in desk, and they will direct you.


ENHANCED VACCINE CERTIFICATE


You may visit covid19.ontariohealth.ca to obtain your enhanced vaccine certificate. Each enhanced certificate contains a QR code that can be saved to your phone or printed as a paper copy. You will be required to present this certificate alongside a piece of ID to verify your vaccination status for certain settings.


Individuals can also call the Provincial Vaccine Contact Centre at 1-833-943-3900 to obtain the enhanced vaccine certificate by email or mail.
