Completed Form DS-11 printed or filled out in black ink. Do NOT sign your form before the appointment. ID must have an identifiable photo. If applying with a minor (under 16), both parents must be present and have valid identification. If both parents can not be present, additional documentation is required. Minors ages 16 and 17 may submit their own identification or have one parent present to present with valid identification.
