·        If you are married, you must submit a copy of your certified and recorded marriage certificate.   If you do not have this document, you may submit your pension application while you work to obtain it. 

·        You must provide proof of your date of birth and, if you are married, your spouse’s date of birth.  To prove date of birth, submit a certified copy of your (and your spouse’s) birth certificate.  Other documents may be acceptable for this purpose, contact the Trust Fund Office prior to your appointment.

·        If you are divorced, you must provide a copy of your Dissolution of Marriage (divorce decree) for each divorce unless you have already done so. If a portion of your benefits were awarded to your ex-spouse, a Qualified Domestic Relations Order (QDRO) will also need to be provided. If you have questions regarding this, please contact the Trust Fund Office prior to your appointment.
