Welcome to the Owen Sound Hospital COVID-19 Assessment Centre Booking System. 

The Owen Sound Assessment Centre is located at 1100 16th Avenue East, Unit C, in Owen Sound (the Sun Life building north of the hospital).

Please note that appointments are required. To book an appointment click the "Book Now" button above.

COVID-19 Testing
You are eligible for COVID-19 testing if you:

are at risk of severe illness from COVID-19; or you live and/or work in high-risk settings, identified as:

                -Hospitals
                -Long-Term Care
                -Retirement homes
                -Congregate living settings

Note: COVID-19 testing for international travel is not offered at GBHS Assessment Centres, as per provincial government direction.  We do not have the ability to provide the required travel documentation. If you are planning a trip, check the COVID testing requirements with the country you are travelling to, as well as requirements for re-entry into Canada:  https://travel.gc.ca/travelling/health-safety/travel-health-notices/221

Important Information:

The test takes five minutes per personAt this time, results take about four days to be returnedDo not arrive more than 5 minutes prior to your appointmentBring and wear your mask

COVID-19 Clinical Assessment
In addition to testing for COVID-19, some individuals will qualify for a clinical assessment. These assessments are only available at our Owen Sound Assessment Centre. You can book a clinical assessment if you know or suspect that you have COVID-19 and you meet the following requirements:

you have been directed by your primary care provider or other health care professional; oryou are unable to safely monitor your symptoms at home; oryou are at higher risk of severe illness, including:

               - immunocompromised individuals aged 18 and over regardless of vaccine status
               - unvaccinated individuals aged 60 and over
               - unvaccinated First Nation, Inuit and Métis individuals aged 50 and over
               - unvaccinated individuals aged 50 and over with one or more risk factors
