Mandatory Consultation Form (All Clients)All clients are required to complete a consultation form prior to their appointment.This applies to every service, without exception.The consultation form allows us to:Understand your hair history and current hair conditionIdentify any allergies, sensitivities, or previous chemical treatmentsAssess the suitability of your selected serviceEnsure all health, safety, and professional standards are metPlease note:Failure to complete the consultation form may result in your appointment being delayed, adjusted, or cancelled.2.
