This appointment type is not for obtaining assistance with completing the TB screening form. If you have not completed the TB screening form, EHS will not contact you. Please contact your manager for assistance or reference the Employee Health page for instructional videos related to completing the TB screening form.
This appointment type is only for employees who respond “YES” to a TB risk or TB symptom question during annual TB screening.
You must complete a telephone visit with an Employee Health nurse for further evaluation, and testing if indicated.
