IMPORTANT INFORMATION REGARDING YOUR APPOINTMENT:·        If you are married, you must submit a copy of your certified and recorded marriage certificate.   If you do not have this document, you may submit your pension application while you work to obtain it.  ·        You must provide proof of your date of birth and, if you are married, your spouse’s date of birth.  To prove date of birth, submit a certified copy of your (and your spouse’s) birth certificate.  Other documents may be acceptable for this purpose, contact the Trust Fund Office prior to your appointment.·        If you are divorced, you must provide a copy of your Dissolution of Marriage (divorce decree) for each divorce unless you have already done so. If a portion of your benefits were awarded to your ex-spouse, a Qualified Domestic Relations Order (QDRO) will also need to be provided. If you have questions regarding this, please contact the Trust Fund Office prior to your appointment.

IMPORTANT INFORMATION REGARDING YOUR APPOINTMENT:If you will be submitting an Electronic Deposit Authorization Form for the electronic deposit of your monthly Pension benefit, you will also need to provide a voided check along with your completed form. We have photo booth rentals available in both So Cal and the SF Bay Area. We require a 2hr minimum and we send two photo booth attendants to manage the booth that we call our "hype men" and we always send tons of fun props! We zoom in and out of each photo to ensure we get the best photos for your photo session and you can customize what you would like the bottom of the photo booth strip to say.
