Welcome to the COVID-19 vaccination service for Unity East PCN. 

Our surgeries include:

Mayfield Medical Practice (Mayfield / Bushbury) / IH Medical Practice / Bilston Health Centre / Probert Road Surgery / Poplars Medical Practice / Ashmore Park / Primrose Medical Practice. 

You will need your NHS number and a valid mobile phone number to proceed.

Your NHS number will be sent to you by your GP surgery but can also be found on hospital letters, on a prescription or on the Patient Access/NHS app(s).

Please book only ONE appointment per NHS number. You will receive a text confirming booking details. If you need help – please call your surgery.

A therapy report is generated from your CPAP device. This can be forwarded to your doctor and explained to you by our sleep consultant

Our sleep consultant will fit and assist in the selection of the appropriate mask for your therapy

Select this option if you are eligible for a free NHS flu vaccination. Those eligible are:

50 years old or over* (over 65’s please see note below)

pregnant

have certain medical conditions

frontline health or social care workers

living in a long stay residential care home or another long stay care facility

a carer for an elderly or disabled person who's welfare may be at risk of you fall ill

*If you are over 65, please note that our clinics are offering the Flucelvax vaccine, which although is not the first choice vaccine for someone over 65 and may be slightly less effective than the first choice, still offers a significant level of protection against flu and is the recommended vaccine for all other adults. Flucelvax can be considered for patients aged 65 or over where the first choice vaccine is not available in your area, and/or you would otherwise go without a flu vaccination this season.
