About the company

PARKING: The parking lot extends from the front of our hospital to the rear of the hospital. There is no charge for parking. Please bring your ID and insurance card, if available. ENTRY for the COVID-19 vaccine is located at the REAR of the hospital. EXIT after the vaccine is located at the hospitals FRONT ENTRANCE. This is NOT a drive through vaccine clinic. You must exit your vehicle and enter at the REAR entrance of the hospital. If you need assistance ambulating or with translation, please ensure you have someone accompany you to receive your vaccine. YOU MUST HAVE AN APPOINTMENT TO RECEIVE THE VACCINE AT MEMORIAL HERMANN SURGICAL HOSPITAL FIRST COLONY. IF YOU FAIL TO SCHEDULE AN APPOINTMENT PRIOR TO ARRIVAL, YOU WILL BE TURNED AWAY. The Johnson & Johnson Janssen COVID-19 vaccine is one dose. You will be required to wait 10-15 minutes for monitoring after receiving your vaccine. Please review the EUA fact sheet for recipients and caregivers - emergency use authorization (EUA) review and sign up for v-safe prior to arrival. Please print out and complete the vaccine consent form PRIOR TO ARRIVAL.
