Wide variety of patients admitted via GP or A&E routes with acute conditions. The acute medical cases represent the hard core of acute and emergency medicine and it is very relevant for your stage of training, so it is strongly recommended that you get involved.

ASU is the acute assessment bay for the newly admitted surgical patients, accepting patients mainly from GP and A&E referrals. You will be able to see and get involved in acute surgical cases such as: appendicitis, pancreatitis, diverticulitis, perforations of bowel, bowel obstruction and many other acute surgical pathologies. Your duty will mainly be shadowing the F1/SHO on call, clerking patients, taking blood from patients, helping the on call team in assessing the new admissions.

WHAT YOU MIGHT SEE/DO AT THE UNIT:
A member of the department will look after you at all times to let you see the correct techniques and procedures when performing venous sampling to ensure that you are competent to then carry out practical.  The department is generally really busy so it will give you plenty of patients to practice on.  We also have patients with PICC lines that are booked in so it will give you the opportunity to observe the correct procedure in which bloods are taken from central lines.

BRIEF DESCRIPTION:
Cardiac Catheter Suite is where Diagnostic Coronary Angiograms take place to determine appropriate treatment plans for patients with chest pain history. Treatment plans include referral for further intervention. Permanent pacemaker implant and complex device implants also take place for patients with heart rhythm disturbances and heart failure.

BRIEF DESCRIPTION:
We see inpatients that have had a recently formed stoma, patients that are inpatients with long standing stomas and have a stoma clinic for troubleshooting concerns with long standing ostomists. We also look after patients who have had bowel resections for cancer and their on-going care.
 
We provide community support for these patients, as well as seeing them in endoscopy at point of suspicion of a cancer, at consultant clinic appointment to break bad news and in follow up after resection.
 
WHAT YOU MIGHT SEE/DO AT THE UNIT: Every day is different, but you will have the opportunity to see ileostomists and colostomists and help care for the stomas of these patients.

BRIEF DESCRIPTION:
The Palliative Care team within the hospital is made up of 8 specialist Nurses supported by senior Doctors from the Rotherham wide Palliative Care Team.  The doctors tend to visit the hospital for 3 morning sessions a week.  The Palliative Care Team works in an advisory capacity and so the patients remain under their own treating team.  We seek to provide holistic assessment and care for those affected by life limiting illness.  We are often asked to support treating teams with issues around symptom management, emotional support and care planning for post-discharge.

WHAT YOU MIGHT SEE/DO:
As a team we discuss patients in the form of a board round and then the Doctor will assess the most complex patients wherever they are in the hospital and offer advice to the patient's own treating team.  You may get the opportunity to talk with patients about their symptoms, perform aspects of physical examination and discuss management plans.  We will often only see 2 - 3 patients during each session, but we will take a holistic approach and consider aspects of physical, social, emotional and spiritual wellbeing.

Some of the patients we support are extremely unwell or may be in their last days of life.  In this situation, students may be exposed to patients and families who are emotional distressed and this will offer opportunities to reflect on communication skills and how we support the dying process.

WHAT YOU MIGHT SEE/DO AT THE UNIT:
A member of the department will look after you at all times to let you see the correct techniques and procedures when performing venous sampling to ensure that you are competent to then carry out practical.  The department is generally really busy so it will give you plenty of patients to practice on.  We also have patients with PICC lines that are booked in so it will give you the opportunity to observe the correct procedure in which bloods are taken from central lines.

WHAT YOU MIGHT SEE/DO AT THE UNIT:

Students will have the opportunity to see patients with chronic vascular disease and leg ulcers. Students will  get an overview of vascular surgery, clinical history taking, vascular disease and learn to examine patients with peripheral arterial, and venous disease (arterial and varicose vein examination). You will have the opportunity to learn how to assess the peripheral circulation with clinical examination and use of hand-held Doppler. Also learn about additional investigations that are frequently required and management/treatment options for patients seen in the clinic.

BRIEF DESCRIPTION:

You will accompany one of the pain nurse specialists/pain ACP or pain consultant on the round to review patients within the hospital -  this could be any inpatient area, including paediatrics  and UECC (we don’t have an outpatient chronic pain service in Rotherham).

We routinely review patients who have epidural, PCA, local anaesthetic infusion, spinal morphine/diamorphine post op, patients with complex analgesic requirements and any ‘live’ referrals made. We will explain/discuss the rationale for specific analgesic regimes, what is appropriate and what isn’t, current national recommendations, local guidelines, best practice. The different treatment options re acute, acute on chronic and chronic pain presentations.

BRIEF OVERVIEW / WHAT YOU MIGHT SEE:
ASU is the acute assessment bay for the newly admitted surgical patients, accepting patients mainly from GP and A&E referrals. You will be able to see and get involved in acute surgical cases such as: appendicitis, pancreatitis, diverticulitis, perforations of bowel, bowel obstruction and many other acute surgical pathologies. Your duty will mainly be shadowing the F1/SHO on call, clerking patients, taking blood from patients, helping the on call team in assessing the new admissions.
